Yes, if the birth took place in other states in Malaysia, the completed EFS application form and supporting documents must be submitted to the Headquarters of the National Registration Department, State of Sarawak within one year from the date of birth of the child. Required supporting documents are as follow:
- A copy of the mother’s or father’s MyKad with Sarawakian ‘K’ status certified by a JPN officer; and
- Copy of baby's birth certificate verified by JPN officer.
Yes, for births abroad, the birth must be registered at the Malaysian Embassy to confirm that the parents have chosen Malaysian citizenship for the child before sending the application to the National Registration Department, Sarawak within 1 year. Please attach a copy of the baby's birth certificate that has been certified by the NRD officer, a copy of the parent's MyKad with ‘K’ status that has been certified by the NRD officer or a copy of mother/father (with K status) biometric printout that has been certified by the NRD officer:
- Full completion of the EFS application form;
- A copy of the mother’s or father’s MyKad with Sarawakian ‘K’ status; and
- A copy of the baby’s birth certificate or Form K.
