- There is no age limit
- Sarawak MyKad with "K" status and residing in Sarawak
- Not civil servants, civil retirees and SOCSO contributors
- Belong to the B40 or M40 group with income not exceeding RM1020.00 per month or according to the current poverty line level (PGK); and
- Does not receive treatment subsidies and transportation assistance from any government agencies, private and non-governmental organizations; or
